Driver’s Hiring Procedure (before start working)

 

PART – I         Request and Copy of Documents

 

  1. Driver’s License should be Class A (view sample)
  2. Medical Card (Green) should be unexpired (view sample)
  3. Valid Truck Registration (view sample)
  4. Copy of SSN and fill-up W-9
  5. Liability Insurance of at least $1 Million Liability Coverage (view sample)
  6. Latest Driving Record from DMV
    1. Fill up Driving Record Authorization
  7. Driver's Road Test (Record / Certificate)

 

PART – II        Read and Sign Documents

 

  1. Application for Sub-Hauler
  2. Request for Employment History
    1. Fill up Previous Employment Verification
  3. Fill up Carrier and Sub-Hauler’s Agreement
  4. Explain and have driver’s initials of the Company Drug and Alcohol Policies
  5. Send to Drug Testing Company and get NEGATIVE Result (view sample)
  6. Enroll the Driver to DMV Employers Pull Notice (EPN)

Maintenance of Driver (while working)

1. Ask and Check the LOGBOOK every payday (weekly).

2. Watch for the 90 days inspection and pre-emptive truck maintenance.

3. Watch the expiration of the submitted documents during application. Get new all the time.

4. Read, sign with date received and put in file the DMV EMP Notices
